The SVH Skilled Nursing Extended Care or
Swing Bed Program
is designed to meet your needs when you no longer require acute care but need more skilled services before you can care for yourself at home.
Located at the hospital, the SVH Swing Bed Program enables you to receive skilled rehabilitation in your own community. After an evaluation by our licensed professionals, you receive daily therapy to meet self-care goals set by you and your Swing Bed team. You are encouraged to wear your own clothes. Your provider will monitor your progress and visit you throughout your stay.
Members of the Swing Bed team meet with you and your family to discuss rehabilitation goals and discharge plans. When those goals have been met, you will be discharged from the program – either to your home or to another facility for long-term care.
Our Swing Bed Services offers:
Physical therapy to restore strength, endurance and mobility; relieve pain; improve movement and function to maximize safety; and rebuild confidence while you adapt to physical changes.
Occupational therapy to promote independence through programs aimed at improving your ability to carry out activities of daily living and restores prior level of function; occupational therapy focuses on fine motor skills, adaptive equipment and activities that foster independence.
Speech therapy helps to regain or improve speech and communication and helps to manage feeding and swallowing problems.
Individual and group activities to promote independence and assist in returning to daily life.
Respiratory therapy to help manage symptoms of breathing problems (asthma, emphysema, etc.) by teaching breathing techniques and exercise, as well as use of medication, including oxygen and nebulizers.
Nutrition therapy to monitor intake of meals, supplements, tube feedings, and intravenous nutrition; evaluate adequacy of energy and protein intake to enable healing, response to stress and recovery; offer education on medication/food interactions and nutrition management of chronic illnesses (diabetes, cardiac disease, COPD).
Nursing to provide daily ongoing assessment and treatment to restore you to optimal health.
Social work to provide assistance with discharge planning and offer supportive counseling to help meet other identified needs; social workers are available to assist with advanced directives, such as Living Wills and Healthcare Power of Attorney forms, and to help residents and families cope with caregiving needs.
Comfort care to support terminally ill patients and their families.
